සේවක අර්ථසාධක අරමුදලෙහි සාමාජිකයින් ලියාපදිංචි කිරීම සඳහා වූ පෝරමය A4 කඩදාසි භාවිතා කර පිටුවේ දෙපසම වන පරිදි (Both side) A4 කඩදාසි දෙකකට මුද්‍රණය කරගතයුතු වන අතර අවශ්‍යනම් මෙම පෝරමයේම අදාළ තොරතුරු ඇතුලත් කර මුද්‍රණය කිරීමටද හැකියාව ඇත.

*කඩදාසියේ දෙපස මුද්‍රණය කර නොමැති පෝරම ප්‍රතික්ෂේප වන බව කාරුණිකව සලකන්න.

ஊழியர் சேமலாப நிதியத்தில் அங்கத்தவர்களைப் பதிவு செய்துகொள்ளும் பொருட்டு படிவங்களை A4தாளில் அச்சிட்டுப்பெற்றுக் கொள்ளும் போது தாளின் இரண்டு பக்கங்களையும் பயன்படுத்தவும் தேவை எனின் உரிய விபரங்களை படிவத்தில் பூரணப்படுத்தியதன் பின்னர் தாளின் இரண்டு பக்கங்களிலும் அச்சிட்டுப் பெற்றுக் கொள்ள முடியும்.

*இரண்டு பக்கங்களிலும் அச்சிடப்படாத விண்ணப்பப் படிவங்கள் நிராகரிக்கப்படும் என்பதனை கவனத்தில்கொள்ளவும்

The form for registration of the members of the epf should be printed in two A4 sheets in both sides by using A4 sheets and on requirement the details can be included into this form and then printouts can be taken.

*Please take into kind consideration that forms which are not printed in both sides will be rejected.
